Please find below Dr. Chitters new patient packet. Please return all forms, as well as a copy of your insurance card. Benefits are checked before this information is sent up to the office for scheduling. Not including your insurance card will only slow down the process. If you cannot email or fax a copy of both the front and back side of your card, please find the form below which lists the necessary information off of your card that I will need.
New patients are typically scheduled within 2 weeks of submitting their information.
Do not drop off your paperwork at Dr. Chitters office. There is no one at the office to receive your paperwork, and this will only cause a delay. The paperwork gets returned to the billing office, so please use the contact information below to return all pages of paperwork.
